A thorough, three-visit evaluation that gives you a clear diagnosis, practical strategies your child can use right away, and the paperwork school needs. Includes:

How it works
Before we begin — intake at home.
We send secure forms and initial screening and rating scales; we score them before your first visit.
Visit 1 — History & assessment.
We review your child’s history and current symptoms and complete the executive-function and initial ADHD assessments.
Visit 2 — Skill-building (about 30 minutes).
Your child actively practices executive-function strategies they can use at home and school right away. During this skill-building session, the provider continues to assess and evaluate for ADHD.
Visit 3 — Diagnosis & plan.
We walk you through the findings and diagnosis, discuss treatment options, and provide a written summary and a school-ready 504 letter (produced after the plan is decided upon).

Insurance & pricing
How payment works
Billed to your insurance.
We’re in-network with Aetna, AmeriBen, Anthem, Blue Cross Blue Shield, Banner Aetna, Cigna, Curative, GEHA, Personify, UnitedHealthcare, and UMR. For covered families, we verify your benefits and bill your plan; you’re responsible for your plan’s copay or deductible.
Self-pay — $995 all-in.
If you’re uninsured, out-of-network, or prefer not to use insurance, the comprehensive evaluation is a transparent $995 — three visits, clinician-scored testing, a written report, and a school 504/IEP letter. We accept HSA/FSA and all major cards.
Need it fast? Optional Priority service.
On a school, testing, or legal deadline? For an additional flat fee, we provide priority scheduling (often within about 72 hours), rush coordination of rating scales once back from teachers, and a fast-tracked evaluation report. This optional administrative service can be added whether you use insurance or self-pay.
Do you take AHCCCS, Medicaid, or Medicare?
Unfortunately, we’re not able to accept those plans. We truly wish we could and have tried to explore participating, but those programs generally do not contract with smaller independent practices like ours. You may want to check with community mental health centers in the area, such as Resilient Health, La Frontera, or Copa Health, as they often accept those plans.

Available in person in Mesa and by telehealth across Arizona. If stimulant medication may be part of treatment, new patients complete a single in-person visit at our Mesa office first; families who prefer to stay virtual can begin with non-stimulant options.

Yes — every evaluation includes a written summary with the diagnosis and treatment plan, plus a school-ready 504/IEP support letter.
- What does it cost, and do you take insurance?
